Over the years, we’ve teamed up with other faith-based organizations to enhance our service to you.

For more than 40 years, Avinity, formerly known as Twin City Christian Homes, has been developing and managing senior living communities throughout the Twin Cities metropolitan area. In 2012, Avinity expanded its senior living options into greater Minnesota. Avinity affiliated with Presbyterian Homes & Services in 2019. Avinity combines extensive management experience with a compassionate faith-based environment to create exceptional communities where aging is all about living well.

Living Hope Presbyterian Church

Living Hope Presbyterian Church participates in a unique joint venture with PHS’ Dickson Hollow senior living community in Menomonee Falls, Wis. The church shares space with the senior living community for Sunday and weekday worship services as well as other programs, performances and events.

Mount Carmel Bluffs, a shared ministry of The Sisters of Charity of the Blessed Virgin Mary (BVM) and PHS, is located on the scenic bluffs of the Mississippi River in Dubuque, Iowa. BVMs are a community of religious women whose members, in response to a personal call from God, have chosen a vowed life of prayer, ministry, and community.

Serving older adults is our passion. Presbyterian Homes & Services made the decision to incorporate an in-house project development team that would serve both PHS and other organizations serving older adults. Senior Housing Partners (SHP) was formed to guide Presbyterian Homes in the pursuit of expanding our mission to more older adults in years to come.

Shepherd of The Lake

Shepherd of the Lake Lutheran Church joined with PHS to build and open McKenna Crossing in Prior Lake, Minn. Both the church building and McKenna Crossing are part of the Shepherd’s Path campus. The congregation now supplies chaplaincy services to McKenna Crossing and many of the residents living there maintain ties to the congregation.

St. Andrew’s Village was developed through the inspiration of St. Andrew’s Lutheran Church in Mahtomedi, Minn., as a shared venture with PHS. St. Andrew’s mission is to inspire, inform, instill and involve the priesthood of believers through prayer, devotions, sharing of God’s work and spreading the love of Jesus Christ.

Sisters of St. Joseph of Carondelet and Consociates St. Paul Province

The St. Paul Province of the Sisters of St. Joseph of Carondelet (CSJ) shares in equal ownership of Carondelet Village, located in St. Paul, Minn. CSJ demonstrates a commitment to continuing Jesus’ mission in the world, by responding to the needs of our time. Strength for this mission comes from fidelity to community, prayer and the practice of “sharing the heart.”

Synod of Lakes & Prairies

PHS shares a covenant relationship with the Synod of Lakes & Prairies of the Presbyterian Church (USA). The synod played a historic role in PHS’ founding and has continued engagement through ex-officio representation on the PHS Board of Directors.

Nonprofit organizations and associations

PHS is active in many local and national organizations and associations that provide pathways to educational opportunities, leadership development and public advocacy to strengthen and broaden our service to older adults.

The Presbyterian Association of Homes and Services for the Aging (PAHSA) is an independent, nonprofit association representing organizations that provide residential communities, health care facilities and services for older adults. PHS is a member and supports the association by providing executive leadership and management services.

LeadingAge

PHS is a member of LeadingAge, a national association of nonprofit organizations serving older adults through adult day services, home health, community services, senior housing, assisted living residences, continuing care retirement communities and nursing homes.

LeadingAge Iowa

LeadingAge Iowa is the state association serving nonprofit and mission-driven organizations dedicated to providing quality housing, health, community, and related services to Iowa’s seniors.

LeadingAge Minnesota

LeadingAge Minnesota is the state’s largest association of aging services organizations with more than 1,000 member organizations including 700 provider member sites. LeadingAge members, including PHS, are diverse but share a common focus on person-directed living, missions of service to their communities and choice in older adult services.

LeadingAge Wisconsin

LeadingAge Wisconsin is a statewide membership organization of nonprofit corporations principally serving seniors and persons with a disability. LeadingAge Wisconsin members, such as PHS, demonstrate a caring commitment to the highest quality of life and greatest independence for each individual served.

Health care organizations

Local and regional health care providers share ownership of and provide service to select PHS communities.

Genevive

Genevive is a geriatric medical practice and care management organization co-owned by PHS, Allina Health and Cassia, providing primary care for older adults, including residents in PHS care settings.

In 2022 PHS partnered with Radius Living Rx, a geriatric-focused pharmacy incorporating genomic testing and integrating “big data” to form optimal health and economic outcomes for older adults.
