Optage Home Care

At Presbyterian Homes & Services, we offer home care services through our own care provider Optage® Home Care—a non profit, faith-based, Medicare agency. Optage provides a compassionate approach to health care encompassing your spiritual, physical, emotional, and social needs. Our goal is to help build a “bridge to optimum aging” through services and programs designed to support your independence.

Your interdisciplinary service team is made up of individuals who bring their unique skills and experience to you in the comfort of your home. Through collaboration, they provide high quality, comprehensive home care. And the services you receive can easily be adjusted if and when your needs change.


In coordination with your physician and other practitioners, nurses evaluate your needs through clinical assessment and then follow through with prescribed treatments and ongoing supportive services. These may include case management, assessment and evaluation, education and support, pain management, medication management and wound treatment.

Nail care services are offered in special clinics at select locations. For feet, service includes foot soak, nail trimming, callus care and lotion. For hands, service includes nail soak, nail trimming and filing.

Private pay fee: Nurse visit: $113/visit | Medication set-up: $64/under 30-min. visit
Medication administration injections: $64/under 30-min. visit | Nail care services: $32/Feet; $22/Fingernails.


We offer weekly home health aide services (assistance with activities of daily living, such as bathing, dressing, grooming, walking and transfer assistance, incontinence care, blood pressure, pulse, respiration and weight monitoring) when a physician requests skilled care for a limited time for a new medical need. 
If you have different home health aide needs, Optage can connect you with a preferred agency for assistance. 


Physical Therapy
Physical therapy helps to restore movement and prevent further disability while promoting independence and may focus on fall prevention, balance training, home safety assessment, walking, instructions in the use assistive devices (canes, walkers, wheelchairs), climbing stairs or managing uneven surfaces, moving from one surface to another (transfers), pain control, and exercises for balance, stretching, and strengthening.

Occupational Therapy
Occupational therapy helps to maximize a person’s endurance and ability to safely carry out their daily routine and may focus on self-care such as dressing, grooming, bathing, eating or meal preparation and the movement necessary during self-care for toileting or getting in and out of tub or shower. Therapists may also concentrate on the use of adaptive equipment and/or adaptive therapies that compensate for physical limitations; strength, endurance and upper extremity coordination needed to perform daily tasks; and problem-solving and safety awareness in the home setting.

Private pay fee: $113/visit
Speech-Language Therapy
Speech-language therapy specializes in communication and swallowing disorders. Speech-language treatment may focus on improving speech quality—louder or clearer, written and adaptive (picture boards) communication, reading  comprehension, and safe swallowing strategies.

Payment Options
Medicare, Medicaid, HMO, private insurance and private pay may cover the costs for Home Care. Private pay Home Care services are also available with a $115 initial assessment/admission fee. Prices are subject to change.
