PHS joins Allina Health as co-owner of Genevive

Allina Health and Presbyterian Homes & Services (PHS) have announced their newly formed collaboration in Genevive, effective January 1, 2019. Genevive is a geriatric medical practice and care management organization providing comprehensive care for 5,500 older adults throughout the Twin Cities and greater Minnesota. PHS will acquire North Clinic’s 50% share of Genevive, while Allina retains their 50% ownership interest. This move positions Genevive as a progressive new model for senior health care delivery between Allina, one of the state’s largest non-profit health systems, and PHS, one of the largest non-profit providers of senior housing and services.

Genevive CEO Amanda Tufano noted, “The mission of Genevive arose from a vision of providers at North Clinic to establish a practice entirely focused on meeting the needs of older adults. This is an incredible opportunity for us to partner with Allina and PHS and further develop what will likely be a national model of care for older adults.”

For 15 years, Genevive has been a leader in providing care through value-driven Medicare and Medicaid contracting, due largely to their geriatric clinical model designed to serve older adults with complex needs. Genevive has successfully made the leap from fee-for-service to value-based care and has a proven track record of improving quality outcomes.

We provide housing and services that represent a significant portion of the determinants of overall health and Genevive has demonstrated success in bringing exceptional care to older adults through its unique combination of primary care home visits, care management, and value-based contracting.”

Allina and PHS have been collaborating to think differently about a next generation of services for older adults modeled after their needs and desires. The two organizations launched a state of the art transitional care model at Interlude Restorative Suites™ in 2015. The collaboration created the unique Interlude transitional care brand that offers innovative care to those who desire rehabilitation with hotel-like accommodations.

Allina and PHS aim to build on Genevive’s strengths and provide care and services in congregate living settings including independent living, assisted living and long-term care. By accessing advanced payment models, including Medicare populations served through Medicare Advantage (MA), Special Needs Plans (SNPs), and dual eligible seniors served through Minnesota Senior Health Option (MSHO) contracts, the partners will better coordinate health care services traditionally offered by hospitals with those services offered by senior living providers.

“This collaboration with Allina in Genevive is the next step in our evolution to focus on ‘living’ and provide a new level of life enrichment services,” says Lindh. “By taking financial accountability through MA, SNP, and MSHO contracts, we believe we can expand wellness and other supportive services for a growing number of older adults, many of whom cannot afford the services and supports that lead to better living and have been proven to reduce the rate of unwanted invasive medical care.”
