Presbyterian Homes & Services and Allina Health have announced that Cassia is joining their collaboration in Genevive. Genevive is a geriatric medical practice and care management organization providing comprehensive care for over 7,000 older adults throughout the Twin Cities and greater Minnesota.

This move strengthens Genevive as a progressive model for senior health care delivery between Allina Health, one of the state’s largest non-profit health systems, and PHS and Cassia, two of the largest non-profit providers of senior housing and services.

PHS, Cassia, and Allina Health aim to build on Genevive’s strengths and provide care and services in congregate living settings including independent living, assisted living and long-term care. By accessing advanced payment models available to Medicare populations served through Medicare Advantage (MA), Special Needs Plans (SNPs), and dual eligible seniors served through Minnesota Senior Health Option (MSHO) contracts, the partners will better coordinate health care services traditionally offered by hospitals with those services offered by senior living providers.
